Health, life, dental, and vision insurance are available to all full-time active employees and their eligible dependents. Insurance changes, enrollment and cancellation are done during Open Enrollment or because of major lifestyle changes. Open enrollment is October 1 - October 31, with an insurance effective date of January 1; no penalty for late health enrollment; no penalty for late dental enrollment; there will be a form of insurability for late life additional coverage. Major life style changes include the following:

• Marriage
• Birth of a child
• Adoption or gain of legal custody of a child
• Divorce
• Spouse loses coverage at place of employment
• Death

All major life style changes MUST be made within 30 days of the event.
